We are seeking a CARE COORDINATION ASSISTANT in Wausau, WI at Aspirus Wausau Hospital! The Care Coordination Assistant is responsible for providing support services to the Aspirus System Care Coordination clinical and SW teams.
The focus of the role is assisting with clerical and administrative work, data entry, tracking and trending and coordination of activities supporting Aspirus patients across the continuum of care.
As part of the multidisciplinary team, the Care Coordination Assistant works closely with physicians, nursing, and ancillary departments to effectively support the Care Coordinator with the patient's plan of care.
The Care Coordination Assistant requires independent judgment, analytical, problem solving, and collaborative skills in order to organize and prioritize multiple tasks to provide patient centered care.
Experience/ Qualifications High School diploma or equivalent REQUIRED Background in healthcare REQUIRED Certified Medical Assistant (CMA) PREFERRED Minimum of three years of experience in an acute or ambulatory healthcare setting.
Previous care management, care coordination, utilization review or population health experience a plus.
Hours:
Full-time or 0.
8 FTE, 64 hours per two weeks pay period.
Day shift hours, M-F alternating varied days of the week.
