Purpose of the Position The Protective Services Representative is responsible for coordinating necessary actions for the appointment of a guardian and protective services or placement, including required evaluations, organizing follow up, completing necessary court reports for cases on assigned case load and conducting adult/elder abuse/neglect investigations.
Education and Experience Requirements Required:
Bachelor's degree in Social Work or related field, earned from an accredited four year college or university academic program.
One to three years relevant work experience Ability to obtain social work certification within six months of hire Valid Wisconsin driver's license and appropriate auto insurance coverage as required by North Central Health Care.
Must have the use of a personal vehicle.
Preferred:
Current Wisconsin Social Worker license Three to five years work experience in Social Work Essential Duties and
Responsibilities:
Assesses and evaluates client's mental status and functioning to determine their appropriateness for Chapters 54 and 55.
Refers to appropriate professionals to complete needed evaluations and assessments.
Recruits, evaluates, and educates guardians and provides ongoing support and assistance.
Collaboratively work with APS team and client interdisciplinary teams formulating recommendations for services and supports for clients.
Utilizes best practice social work skills in assessing client needs and supports Facilitates and mediates for client needs and services - working with Court, Guardians, and service agencies and providers.
Utilizes information gathered, assessments conducted and make decision appropriately.
Utilizes knowledge base of client rights and need to protect in making assessments/determinations of least restrictiveness for court recommendations.
Maintains client's rights to self-determination as appropriate based on their cognitive abilities.
Maintains regular and predictable on-site attendance.
